Job Description
Reporting to the Supports & Services Manager (SSM), the Home Coordinator is responsible for the daily supervision and operation of assigned residential group homes serving those with intellectual disabilities.

ESSENTIAL FUNCTIONS:
The essential functions of this position consist of the ability to perform basic math functions, write, read, and comprehend what is read.  Expressive and receptive language skills are also required in the execution of the responsibilities below. The ability to transfer and lift individuals with or without assistance and to occasionally push/pull may also be required.  Twisting, bending, and other physical movements may be required at varying degrees. All the above may be performed with or without reasonable accommodation.

DUTIES:
●Direct and oversee daily program operations within assigned residential group homes through the supervision of team members, including performance evaluations and management, scheduling, and ODP staff compliance.
●Responsible for visiting, at a minimum, one group home per day and will visit all assigned residential group homes within their responsibility at least once a week.
●Under the supervision of the SSM, participates in interviews, selection, and the onboarding process of new hires
●Ensure services are provided in accordance with agency policies and procedures.
●Direct and oversee medication management, electronic health record documentation and monitoring, medical training for staff regarding individual’s diagnoses and medical conditions, and staff support for health/medical related questions.
●Coordination of medical appointments for individuals including transportation and follow-up.
●Facilitates effective communication within the team, including external individual supports.
●Participate in employee relations activities with the team and engage in disciplinary actions, as needed, with assistance from Human Resources.
●Assist with annual licensing preparation and plans of corrections as needed.
●Partner with leadership team to manage the communications for staff and leadership including annual enrollment guides, new hire guides, etc.
●Coach and mentor direct reports; provide training, oversee performance, and aid with career progression
●Perform other related duties and assignments as directed by the Support and Services Manager. This position will also serve as the backup for the positions supervised.

HOURS:                                                           Non-Exempt, 40 Hours per Week to include one weekend per  month and off-shifts

QUALIFICATIONS:

●Requirements include an Associate’s Degree in human services, social work, psychology, or related field (OR) a High School Diploma or equivalent and two (2) years of experience.
●Demonstrates interest in the development of others and positively impacts the lives of patients/families, peers, and members of the Hope team through mentoring, education, and knowledge sharing.
●Valid driver’s license, required.
