Blended Case Manager Supervisor

ESSENTIAL FUNCTIONS:
The essential functions of this position consist of the ability to perform basic math functions, write, read, and comprehend what is read. Expressive and receptive language skills are also required in the execution of the responsibilities below. Twisting, bending, and other physical movements may be required at varying degrees. Use of telecommunications equipment is required along with the data input and gathering via a computer. All the above may be performed with or without reasonable accommodation. This position requires the ability to move and work within multiple residential and office settings and requires frequent travel to and from client’s residences in order to provide services.
DUTIES:
- Provide direct supervision and training to casework staff through regular casework conferences and treatment team meetings.
- Provide back-up support to casework staff in managing more complex cases, crisis or emergency situations and coverage. Assure Agency clients coordinated access to services and provide transportation to Agency clients when appropriate.
- Conduct regular supervisory and staff meetings to discuss cases and develop treatment plans.
Assist case management staff in conducting individual assessments and developing comprehensive goals and treatment plans. - Schedule case reviews to monitor effective service delivery.
- Participate in interagency planning and collaboration to maximize community resources.
- Participate as the representative in applicable licensing/certification reviews.
- Maintain familiarity with the Management Information System, including computer fluency necessary for effective use of the applicable State and/or Hope computer systems.
- Monitor staff performance in terms of meeting billable unit standards on a regular basis.
- Schedule weekly on-call rotation and back-up coverage for Blended Case Management staff.
- Coordinate time off to ensure that appropriate coverage is always provided.
- Provide ongoing training to staff members to ensure quality of service.
- Assist staff with developing and maintaining an effective working knowledge of suicide awareness and risk assessment through participation in Agency identified or Agency approved trainings.
- Monitor staff’s proficiency in these areas on a regular basis through supervision to ensure staff’s practical application of this knowledge.
- Ensure maintenance of appropriate record-keeping by the staff by reviewing and monitoring client records.
- Review and approve all requests for Respite Services.
- Attend Transition Council meetings when appropriate.
- Maintain contact with identified clients in Residential Treatment Facilities (RTF’s) to assist with discharge planning.
- Conduct searches for facility placements (RTF’s) for identified clients and provide facilities with necessary paperwork for clients’ acceptance.
- Review and evaluate staff performance on a regular basis to ensure the provision of quality services.
- Maintain knowledge of all individual-serving systems in order to assist staff in making appropriate referrals.
- Participate in Clinical Supervisor’s meetings as a forum for identifying internal and external problems related to service delivery and developing ways of addressing those issues.
- Perform other related duties and assignments as directed.
HOURS: 40+ hours per week, Exempt position, flexibility required

QUALIFICATIONS:
Master’s degree in social work, psychology, rehabilitation, activity therapies, counseling or education and 3 years mental health direct care experience;
OR Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; OR be a registered nurse;
OR a high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years’ experience in direct contact with mental health consumers; OR a high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989
Valid driver’s license, required.
