Community Participation Support - Coordinator

The Community Participation Support Coordinator under the supervision of the Director – Community Participation Support (CPS) is responsible for the supervision of services for the Community Participation Support Program.  The Community Participation Support Coordinator is responsible for the direct supervision of Program Specialists CPS.

Identify needs and weaknesses of service delivery.Participate in the development of policies and procedures to improve operation of service delivery.
Promote a trusting relationship and remain professional when working with individuals/families.
Facilitate and maintain relationships between the Administrative Entity, Supports Coordination Organization, day programs, and other support services.
Avoid allowing personal beliefs and values to influence dealings with individuals receiving services.

Supervision
Supervise and provide leadership to Program Specialists-Community Participation Support.
Participate in orientation and ongoing training for Program Specialist-CPS.
Ensure compliance with Everyday Lives, Hope Enterprises, Inc. Policy and Procedures, and current ODP practices and philosophies.
Interview and hire personnel.
Complete and discuss performance evaluations.
Address performance issues and follow up under the direction of the supervisor and ensure consistent application of disciplinary actions in conjunction with agency policies.
Approve work schedules, time records, expense vouchers, and other required forms.
Ensure required operational paperwork is reviewed, approved, and submitted in a timely manner.
Oversee the development of caseloads to ensure consistency in service delivery.
Meet regularly with Program Specialists.
Ensure attendance at required meetings and trainings.

Responsibilities
Develop and implement Community Participation Supports for individuals which adheres to 2380 Regulations and funding requirements.Strive for continuous expansion of community opportunities and arrange for relevant facility or community based activities.
Ensure individuals’ services are administered and maintained in accordance with applicable policies, procedures, regulations, and relevant laws.Monitor individuals’ records for compliance.
Ensure completion of all programmatic requirements.
Have full knowledge of 2380 regulations.Maintain Program Specialist qualifications as defined by 2380 regulations.
Ensure maximum provision of services by monitoring service delivery and unit utilization.
Monitor program budgets, ensure expenditures are consistent with budget limits, and make recommendations for budget changes as necessary.Review purchases and requisitions as appropriate.
Oversee the fiscal status of the program to include individual authorization procedures, projection, accrual, and monitoring of services units.
Assist in the development of program budgets.
Be involved in the referral process and the selection of individuals.
Facilitate and maintain relationships between the individuals, families, and other support services.
Oversee crisis intervention in emergency situations.Notify the Director CPS of the emergency situation.

Overall Operations
Ensure the quality and provision of services by participating in ongoing program audits and ensure licensing and accreditation.
Work with appropriate staff to ensure facilities and equipment are managed and maintained in a safe, operable condition and meet applicable codes and regulations.
Ensure that appropriate program equipment and supplies are purchased within budgetary guidelines.

Notification of Incidents
Have complete knowledge and comply with Incident Management.
Participate and comply in Incident Management meetings.

Other Duties
Make presentations to other agencies, organizations, and interested parties.
Participate in committees as assigned.
Attend and participate in required training sessions, seminars, and/or conferences.
Prepare statistical and other reports as required.
Participate in the completion of Provider Qualifications.
Perform other duties as may be required or assigned.

QUALIFICATIONS: Master’s degree or above from an accredited college or university and 1 year work experience working directly with persons with disabilities or a bachelor’s degree from an accredited college or university and 2 years work experience working directly with persons with disabilities or an associate’s degree or 60 credit hours from an accredited college or university and 4 years work experience working directly with persons with disabilities.
